Intention Health Care is required by law to maintain the privacy of your protected health information (PHI), to provide you with this notice, and to abide by its terms. We treat all PHI with strict confidentiality.
How we use and disclose PHI
We may use and disclose your PHI for treatment, payment, and routine healthcare operations. We may also disclose PHI when required by law, to public health authorities, or to comply with legal proceedings. We do not sell your PHI.
Your rights
You have the right to inspect and copy your PHI, request amendments, request an accounting of disclosures, and request restrictions on certain uses or disclosures. You may also request communications by alternative means.
Filing a complaint
If you believe your privacy rights have been violated, you may file a complaint with us at privacy@intentionhealthcare.com or with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
